What is methotrexate?

Methotrexate is an antifolate and antimetabolite drug that is used to treat a variety of cancers such as leukemia, lymphoma, breast cancer, lung cancer, and neck cancers, among others. It is also used in the treatment of various autoimmune diseases including rheumatoid arthritis, psoriasis, and lupus. Antifolates impede the function of folic acid in the body while antimetabolites inhibit the utilization of metabolites.  The impediment of folates and metabolites interferes with cell production of RNA and DNA, stopping cell division and growth. This is why methotrexate is used as chemotherapy to stop the rapid growth of cancerous cells in the body. This is also why it is a potentially dangerous drug, and it is used to induce abortions. Methotrexate works to treat arthritis because it inhibits the function of T cells in the immune system and their role in causing inflammation.

How do I take methotrexate?

Methotrexate may be administered intravenously in the hospital or purchased as a vial or tablet using or methotrexate coupons in your local pharmacy. If you are self-medicating, follow instructions given to you by your doctor or pharmacist for your condition. Methotrexate is usually taken in doses of three or four tablets at one time once a week. Take your medication with a glass of water. If you are injecting your medication, do not use any vials of discolored or cloudy medicine. Throw it away and replace it. Drink lots of liquids while taking this medication. It may take several weeks to notice the benefit of methotrexate. Finish the full course of treatment prescribed by your doctor. Before taking methotrexate, tell your doctor if:

  • You have lung, liver, or kidney disease.
  • You are HIV positive or have been diagnosed with AIDS.
  • You are allergic to methotrexate or any of the ingredients in this medication.
  • You are pregnant, breast-feeding, or plan to father a child.
  • You have stomach ulcers, ulcerative colitis, or a blockage or inflammation of the bowels.
  • You have immune system, nervous system, or blood problems such as seizures, anemia, low white blood cell count, or low platelet levels.
  • You have a history of drug or alcohol abuse, or have several drinks daily.
  • You have recently had a vaccine.
  • You are taking prescription or nonprescription drugs, supplements, or vitamins, specially antibiotics, NSAIDs, Pepto-Bismol, probenicid, or trimetrexate.
  • You currently have an infection, mouth sores, retention of fluids, or dehydration.
  • You are already receiving chemotherapy or radiation.

What are the possible side effects of taking methotrexate?

Side effects from taking methotrexate include nausea, abdominal pain, low white blood cell count and an increased propensity to infection, fatigue, dizziness, mild hair loss, discomfort, sore throat, menstrual changes, ulcerative stomatitis or inflammation of the mouth, fever or chills, breast enlargement in men, vision changes, vomiting, vaginal discharge, bloody stools or urine, loss of appetite, coughing up blood and dry cough. These are not all the possible side effects from taking methotrexate. Call your doctor if serious or unbearable side effects persist.

What if I forget to take a dose of methotrexate?

If you miss a dose of methotrexate, contact your doctor. Do not take a double dose of methotrexate to make up for a missed one.

How do I store methotrexate?

Store methotrexate in its original container or packaging at a room temperature between 68 and 77 degrees Fahrenheit. Keep this medication away from light and out of reach of children and pets.

What happens if I overdose on methotrexate?

Signs of overdose on methotrexate include black or bloody stools, change in urination, severe headaches and nausea, seizures, unusual bruising or bleeding, unusual weakness or fatigue, vomiting, and swelling and soreness in the mouth and tongue. If you or someone you know has overdosed on methotrexate, go to the emergency room immediately. If the victim is unconscious, call 911.

Warnings

Do not become pregnant while taking methotrexate. Both men and women should practice reliable forms of birth control. Talk to your doctor about the most effective option. It is advisable to use birth control for up to three months after discontinuing use of this medication.
