What is testosterone?

Testosterone is the male hormone, classified as an androgen or endocrine metabolic agent, and is responsible for development of the secondary sex characteristics in men including development and maintenance of sexual organs, facial hair growth, muscle development, and other attributes. In prescription form, it is used to treat male testosterone issues, including low testosterone levels (hypogonadism).

While there are no multiple types of testosterone, there are different formulations of testosterone gel available to meet the needs of any given patient. There is not, however, any prescription testosterone cream available at this time; only gel.

Occasionally, women may be prescribed a compounded formula of testosterone medicine to help with certain problems, but it will be specially formulated based on the patient’s medical needs. However, there are no testosterone gels, testosterone creams, or low testosterone treatment universally approved or available for women.

For transgender procedures, testosterone injections are usually followed by regular administration of testosterone gel to maintain the desired effects.

How do I take testosterone?

Testosterone is delivered as a topical gel, applied using a pump or from a packet, and is available in a number of dosage strengths. It is also available as a transdermal patch. The typical prescription is for once daily application, preferably at the same time each day. The amount to apply and where to apply it depends on the strength of the prescription. There are no approved low testosterone pills.

Regardless of the strength of dosage, the area where it is being applied must be cleaned beforehand. Wash hands thoroughly after application.

Some low testosterone treatment processes may require a serum testosterone injection; your physician will review your medical history to determine if this is the best type of testosterone treatment for you.

If you are using a transdermal patch, follow the instructions that are included with the prescription.

Before taking testosterone, tell your doctor if:

  • You are allergic to testosterone, or any medications, whether prescription or over-the-counter
  • You have an enlarged prostate, you have been diagnosed with prostate cancer, or there is a history in your family of prostate cancer
  • You have limited knowledge of this hormone and want to know “What is testosterone?”
  • You are allergic to alcohol or alcohol-based products
  • You are taking anticoagulants or blood thinners
  • You are taking any medications (prescription and non-prescription), vitamins, or supplements
  • You are taking oral, topical, or inhaled doses of testosterone, steroids, or similar medications
  • You have breast cancer
  • You have heart, liver, kidney, lung disease, or diabetes
  • You are taking over-the-counter testosterone treatment medications marketed as low testosterone pills or supplements
  • You have sleep apnea or other sleep disorders
  • You have children in your home are at-risk to exposure.

What are the possible side effects of taking testosterone?

There are several side effects associated with the use of testosterone. The less severe symptoms include hair loss, dry skin, headache, change in sexual desire or habits, acne, breast enlargement or pain, depression or other mood changes, headache, or the onset of unusual muscle weakness. Contact your doctor if these or any other symptoms persist or increase in severity.

Serious side effects that require immediate medical attention include trouble breathing during sleep, changes in erection frequency or erection duration (especially those that last four hours), nausea and/or vomiting, and swelling of the arms, hands, legs, or feet. Other severe effects include changes in urination patterns (including difficulty, weak stream, urgent need or an increase in frequency), and changes to the skin, especially its color or tone. 

What if I forget to take a dose of testosterone?

If you miss your scheduled dose of testosterone, take it if it is twelve hours or longer until your next scheduled application. If the next dose is due within twelve hours, skip the missed dose and resume the regular dosing schedule. Do not make additional applications of the testosterone gel. 

How do I store testosterone?

Keep testosterone in its original packaging, away from the extremes of heat and humidity. Do not allow the gel to freeze. Dispose of any unused portions according to the recommendations of your physician. Do not expose any low testosterone medication to open flame.

What happens if I overdose on testosterone?

If you overdose on testosterone, contact poison control at 1-800-212-2100. If you or the patient has collapsed or has stopped breathing, contact Emergency Services at 911. If a child or a female has come in contact with low testosterone drugs, seek medical assistance.

Warnings

Although both men and women produce testosterone, male testosterone levels are higher, therefore care must be taken by women and children who may come in contact with testosterone gels. Secondary exposure to low testosterone drugs can cause accelerated virility in children exposed to them. If combined with testosterone, steroids may have additional side effects. Testosterone in gel form is highly flammable. If using the gel, do not cover the medicated area with clothing until it has completely evaporated.

Brand Names

Testosterone is available under a wide range of brand names, but they are not different types of testosterone, only different strengths and forms. Testosterone can be administered as a transdermal, low testosterone patch, a testosterone injection, or a testosterone gel. Brand names for this product include Androderm, Androgel, Axiron, Testoderm, Testia, Fortesta, Vogelxo, First-Testosterone, First-Testosterone MC, Depo-Testosterone, and others. Formulations and strength vary among manufacturers.
