What is ketoconazole?

Ketoconazole is an azole antifungal medication of the class imidazole that is used to treat certain types of yeast or fungal infections such as chromomycosis, thrush (candidiasis), blastomycosis, histoplasmosis, ringworm, athlete’s foot, and others. Ketoconazole is not used to treat fungal infections of the finger or toenails, Cushing Syndrome, or prostate cancer. It is available only by prescription, and is recommended only when other antifungal treatments are not effective.

How do I take ketoconazole?

Ketoconazole is prescribed either in tablet form or as a topical cream or a shampoo. Take the tablet once a day, at the same time every day, with water or other approved liquid. Ketoconazole cream and shampoo are for use only on the skin and scalp. When using the cream, apply enough to cover the affected and immediately surrounding area, and gently massage it in. Wash your hands immediately after.

When using a ketoconazole shampoo, wet the scalp and apply the shampoo to the affected area, and gently massage the medicine to develop a foam. Leave the shampoo on the affected area for five minutes, then rinse thoroughly with water.

Do not let the shampoo or cream reach the eyes. If this happens, flush with plenty of water. Do not ingest the cream or shampoo.

Before taking ketoconazole, tell your doctor if:

  • You are allergic to ketoconazole or any of the ingredients in the medication
  • You are taking any prescription or non-prescription medications
  • You are taking any vitamins or supplements
  • You are taking antacids, especially those containing calcium, magnesium, or aluminum
  • You are pregnant, planning a pregnancy, or are nursing a child
  • You have, or have had, liver disease
  • You have, or have had, heart problems or disease, or a family history of heart disease
  • You have a family history of QT prolongation
  • You have, or have had, problems with your adrenal gland
  • You are taking lovastatin, pimozide, alprazolam, cisapride, midazolam, or ergotamine
  • You are taking triazolam, quinidine, dronedarone, ranolazine, eplerenone, simvastatin, disopyramide, nisoldipine, or dofetilide
  • You are taking medications to treat migraines, depression, arrhythmia, malaria, or mental illness
  • You are taking antibiotics
  • You are allergic to sulfites (when prescribed the cream)

What are the possible side effects of taking ketoconazole?

Those taking this medication have reported side effects. Less severe ketoconazole side effects include male breast enlargement, decreased sexual performance or desire; gastrointestinal problems including constipation, gas, diarrhea, heartburn, and stomach pain; light sensitivity; headache, muscle pain, numbing or tingling in the extremities; sleep problems or changes in sleep patterns; flushing, dry mouth, chills, or nervousness; changes in color of the tongue, the ability to taste food, dry mouth, or nose bleeds; changes to hair texture or loss of hair; or blistering of the skin or scalp. However, if any of these conditions persist or worsen, contact your physician.

Severe ketoconazole side effects requiring immediate medical attention include signs of liver disease (jaundice, upper stomach pain, darkened urine, clay-colored stools, loss of appetite); signs of an allergic reaction (swelling of the lips, face, throat, tongue, or mouth; difficult breathing or swallowing); sudden rash or hives. 

What if I forget to take a dose of ketoconazole?

If you forget to take a scheduled dose of ketoconazole pills, or miss an application of its topical formulation, do so as soon as your remember and are able. However, if the time is near for the next scheduled dose or application, forego the missed one and resume the normal schedule. Do not take multiple doses or apply extra ketoconazole to compensate for what was missed.

How do I store ketoconazole?

Store this medication in its original container out of the reach of children and away from extremes of temperature and humidity. If you are using any of the topical formulations (ketoconazole cream, foam, or ketoconazole shampoo), do not allow them to freeze. Speak with your pharmacist about proper disposal procedures for unused or expired doses of ketoconazole.

What happens if I overdose on ketoconazole?

If an overdose of ketoconazole is suspected, contact Poison Control at 1-800-222-1222. If the individual has stopped breathing or lapsed into unconsciousness, call Emergency Services at 911. If signs of liver disease appear, contact your doctor immediately.

Warnings

Ketoconazole can cause serious liver damage, and may result in the need for a liver transplant. Contact your doctor immediately at the first sign of liver problems. Ketoconazole may cause an irregular heart rhythm known as QT prolongation. Do not nurse a child while taking this medication. This medication may reduce sperm count. Contact your doctor if you become pregnant while taking this medication. Consuming alcohol while taking this medication can exacerbate or accelerate the chances of developing liver disease. Do not eat the shampoo or cream. Do not stop taking or using this medication until approved by your doctor.

Brand Names

Ketoconazole pills are available under the brand name Nizoral. Ketoconazole cream is marketed as Ketozole Cream, Xolegel, and Nizoral Cream; the foam is sold as Extina Foam. Ketoconazole shampoo appears under the brand names Nizoral Shampoo and Nizoral AD Shampoo.
