What is topiramate?
Topiramate is a sulfamate-substituted monosaccharide anticonvulsant tablet for oral administration. It's prescribed to treat seizures and migraines in children as young as 2 (standard release) or as young as 10 (extended release), and works by calming abnormal excitement in the brain by acting on GABA receptors, sodium channels, and calcium channels.
While doctors may not prescribe topiramate for weight loss, it may help with weight loss. You should not take topiramate for weight loss unless your doctor allows you to do so. Your doctor may prescribe topiramate medication for reasons not listed in this article.
How do I take topiramate?
Topiramate can be taken with or without food, but should be taken with a whole glass (8oz) of water. Your doctor will determine your dosing based on what's being treated, your age, and your medical history. Most often the prescription is for 1 pill 2 times daily in the mornings and evenings. Do not crush, chew, or break topiramate pills before taking them. Try to take your medication at the same time every day, and do not take more or less than prescribed.
Before taking topiramate, tell your doctor if:
- You take any other prescribed or over-the-counter medications, vitamins, supplements, or drugs, including: any other seizure or anticonvulsant medications, glaucoma medications, metformin, birth control pills.
- You have any impulsive behaviors.
- You are on a special diet, especially one that's low in carbohydrates and high in fat.
- You are dehydrated or often become dehydrated.
- You regularly drink alcohol.
- You are pregnant or breastfeeding or plan to become pregnant or to breastfeed.
- You are depressed and have suicidal ideations.
- You have or have a history of high acid levels in your blood.
- You have or have a history of eye or vision problems, including glaucoma.
- You have or have a history of serious pulmonary or breathing problems.
- You have depression or another mood or psychiatric problem, and you are taking medications (e.g., lithium) for it.
- You have or have a history of a growth disorder.
- You have or have a history of osteoporosis or osteomalacia.
- You have or have a history of kidney disease or disorder, including if you are on dialysis.
- You have or have a history of liver disease or disorder.
- You currently or recently were ill with diarrhea.
What are the possible side effects of taking topiramate?
Some topiramate side effects can be dangerous and require immediate emergency medical attention. These include: confusion or memory loss, trouble thinking or remembering things, feeling tired or like you might pass out, high fever, unexplained weakness, eye pain or sudden vision loss, hives and itching, hot and dry skin, swelling in the face or lips, trouble speaking, swelling in the tongue or throat, feeling short of breath or difficulty breathing, irregular heartbeats, decreased sweating, severe pain in your side or lower back, severe loss of appetite, vomiting, difficult or painful urination.
You should talk to your doctor immediately if you experience any of the following: panic attacks or other anxiety, aggression, agitation or hostility, impulsive thoughts or actions, hyperactivity (mental or physical), restlessness and trouble sleeping, depression or thoughts of suicide. Also talk to your doctor if your migraines or seizures worsen.
Common topiramate side effects include: feeling warm or looking flushed, numbness and tingling in arms and legs, change in your sense of taste, changes in mood, feeling nervous or tired, mild headache or dizziness, rapid movement of eyes back and forth, mild nausea or stomach pain, indigestion, loss of appetite, weight loss, stomach pain, diarrhea. These side effects do not require medical attention unless they persist or worsen.
What if I forget to take a dose of topiramate?
If you realize you've forgotten 2 or more doses of this medication, call your doctor about how to proceed.
You can take your missed topiramate dose as soon as you remember it, unless it's almost time for your next dose. In that case, skip your missed dose. Always maintain your regular dosing schedule.
How do I store topiramate?
Store topiramate pills on the cool side of room temperature and away from excess heat, light, or moisture (e.g., a bathroom). Always keep the bottle tightly closed.
What happens if I overdose on topiramate?
Symptoms of overdose may include: thinking problems, agitation or depression, problems with speech or coordination, double vision, drowsiness, fainting, or seizures. If you suspect an overdose, call your local poison control center, or seek emergency medical assistance.
Warnings
Discontinuing this medication suddenly may cause an increase in seizures or migraines. Do not stop taking your medication unless instructed to do so by your doctor, who will instruct you on how to taper your doses.
Also, do not take your topiramate ER dose either 6 hours before or 6 hours after drinking alcohol. Drinking alcohol while using topiramate can increase side effects. Anticonvulsants like topiramate may cause some people to become suicidal, so your doctor and family or caregivers should be aware of changes in your mood.
Always stay hydrated and avoid ketogenic diets while taking this medication, otherwise you will experience electrolyte imbalance or may develop kidney stones. Certain other medications can have as similar effect.
Be careful administering it to children. Topiramate may cause metabolic acidosis, which can harm unborn children, cause growth problems in young children, weaken bones, or lead to kidney stones. This medication can make birth control pills or other hormonal birth control less effective. Always use backup birth control such as a condom while taking topiramate. Talk to your doctor if you become pregnant while taking this medication.
Avoid potentially hazardous tasks like driving or operating heavy machinery until you know how you'll react to this medication.

Topiramate is the generic version of the brand name medication Topamax and the extended release Trokendi XR. There are currently no other brand names for topiramate pills.
